1.)On home page, please click on Report an Incident 

 


2.)Key in your email address on the requester column

3.)Key in the Subject (Please indicate the Issue and location)

4.)Key in description of your issue.


5.) Once completed, click on [Submit]. (An registration Email will be send to you if you have not create an Account).


6.)You will receive a confirmation email once you have successfully create a ticket. 



7.)Once the issue is resolved and closed, you will also receive a mail on the status of the ticket (Or you can login with your account and check the status of the ticket).
